I’ve been asked by our Central Safety Committee to gather some information from other Local Authorities who deliver Community Care/Care Home Services on how they report and record Medication Incidents. The Committee also want to benchmark our figures against other Local Authorities to see how we compare. It would also be useful to know what categories your Council uses to define Medication Incidents.

Any information/advice would be very much appreciated.
